The fibroblast growth factor receptor 3 (FGFR3) is one of the most common mutations associated with desmoplastic trabecular tumors and is related to tumorigenesis, especially in soft tissue. The aims of the present study were to evaluate the involvement of FGFR3 gene alterations in the development of leiomyosarcoma and to assess the prognostic significance of this molecular marker. Twenty-one cases of leiomyosarcoma of soft tissues were studied, nine located in the deep soft tissues, nine in the superficial soft tissues, and three in the muscles. By means of the PCR-SSCP technique, 12 (57.1%) cases showed alterations in FGFR3, three of which (12.5%) presented a typical loss of heterozygosity at codon 337. In the three cases located in the muscles, a duplication was identified in exon 9, whereas in the superficial leiomyosarcomas, a missense mutation was detected in exon 10 (codon 659). The mutation detected in the muscle tumors had been previously described in desmoplastic trabecular tumors, whereas the two others were novel. All of the alterations observed in the tumors were heterozygous and associated with a loss of the second allele. The FGFR3 mutations were always mutually exclusive with those of p53 and EGFR. No significant correlation was observed between FGFR3 alterations and tumor grade, stage, and site of origin. The present study shows the first direct evidence that FGFR3 gene alterations may be implicated in the pathogenesis of a subset of leiomyosarcomas of soft tissues. Our data further show that the presence of FGFR3 mutations is a common feature of a subset of leiomyosarcomas and is associated with a poorer prognosis.Acalanthe atropurpurea
